About

This program assists Chickasaw citizens with special needs by providing a yearly reimbursement of up to $500 for participation in extracurricular activities. Required application is available by calling the telephone number above or online at www.chickasaw.net/vocrehab.

You Provide
  • Chickasaw citizenship card/certificate
  • Current IEP (individualized education plan for citizen with
    special needs) or documentation of a disability from a physician
  • Proof of residence
  • Receipts for assistive technology or expenses paid to participate in
    recreational activities
