• Must be in 1st - 12th gradess

You Provide:

• CDIB
• Up-to-date immunization record
• Birth certificate
• Social Security card
• School transcript
• Current contact list
• Copy of insurance, Medicaid or Sooner Care

Required forms are available at the website below or can be mailed upon request. Applications are taken year round until enrollment is full. An applicant should know within three days.
