About

This program provides an allowance of $125 toward contact lenses or one pair of glasses every two years. The eye glasses are made by Oklahoma Optical, a tribally owned lens manufacturing business. Specialty lenses and frames are available at a low cost to the patient.

For members of other tribes, contact your tribal organization for information on payment assistance with eyeglasses.

You Provide
  • Glasses prescription and frame ordering information to include measurements for bifocal height and pupil distance
Eligibility
  • Must be Chickasaw
  • Applicant must be a patient at CNDH
  • Prescription must be signed by a CNDH provider
  • Eligible for one benefit every two years, excluding sunglasses
Other Info

Citizens will send frame ordering information along with glasses prescription to the tribal health program. The ordering voucher will then be issued and the job will be completed to include any specialty items ordered by the citizen at a discounted rate. Completed glasses will be mailed to the citizen. One-day service is offered on most orders.

All available frames in stock and specialty items are available to the citizen from Oklahoma Optical inventory. Citizens living nearby may come to Oklahoma Optical to try on frames and get measurements for their selection. Frame adjustments may be obtained by the citizen from an optical shop or provider of choice.

This $125.00 program voucher will assist to purchase glasses or contact lenses dispensed by Oklahoma Optical and is not a reimbursement plan for eye care items obtained elsewhere.
