Chickasaw Nation violence prevention programs provide advocacy services, shelter, transportation, career and education services, and mental health counseling to victims of domestic violence and their dependents.

• Driver’s license
• Social Security card
• Referral from agency (if applicable)
• Case history/narrative, police reports, protective orders, medical reports
• Statements for requested assistance
• Income verification (if applicable) for case management purpose only
• CDIB (if Native American)

• Must be current or recent victim of domestic violence, sexual assault or stalking
• Residing in or relocating to the Chickasaw Nation
