This is an appointment-based clinic offering outpatient family practice medicine including laboratory testing, drive-thru pharmacy and periodic specialty clinics.

Hours of Operation: Monday - Friday, 8:00 a.m. - 5:00 p.m.
The Pharmacy hours are 9:00 a.m. - 12:30 p.m. and 1:30 p.m. - 5:00 p.m.
(closed Federal holidays)

The Chickasaw Nation Division of Health in Purcell provides a variety of health services.  Services provided include:

Optometry

  • Offer eye exams to adults and children
  • Full eye exams
  • Refer patients to ophthalmologists within the tribe
  • Appointments are on Tuesdays 9 - 4

Laboratory

  • Ambulatory hematology, chemistry, urinalysis, microscopics, routine care
  • Specialty testing via reference lab

Medical Records

  • Customer service
  • Registration of Patients
  • Release of information

Pharmacy

  • Fill prescriptions
  • Counsel on drug information
  • Call Carl Albert Pharmacy (1-800-851-9136 ext 81551) requesting refill needed to be refilled.
  • Call Purcell Pharmacy (405-527-4911) and let them know that you will need your medications to be delivered.  Please leave your name, chart #, and phone #; so that the pharmacy can reach you if there are any questions regarding your prescriptions.
  • Medications should be available 2 business days following transportation request.  However, all patients should call the Purcell Pharmacy (405-527-4911) to confirm delivery prior to medication pick-up.
