About

This program promotes the highest possible health level of Native Americans by providing medically-guided primary health care services where no other resource is available. Services offered by the CHR program include: patient advocacy, liaison between clients and tribal and community resources, patient education and health promotion/disease prevention.

You Provide
  • CDIB or tribal citizenship card
  • Social Security card
  • Proof of residency
Eligibility
  • Must reside within the Chickasaw Nation
