About
Counseling services are available to persons of all ages, including youth, through the Behavioral Health Program of the Chickasaw Nation Division of Health(CNDH). American Indian youth who have a CDIB and are eligible for services through the CNDH may receive counseling services for:

  • Life Adjustment Issues
  • Grief
  • Divorce
  • Behavioral Problems
  • Alcohol/Drug Issues
  • Domestic Violence
  • School/Gang Violence
  • Life Coping Skills
  • Depression
  • Anxiety

Eligibility

  • Persons may be referred by their physician, parent or self.
  • Youth, persons under 18, must be accompanied to their behavioral health visits by a parent/legal guardian.
  • Parent/legal guardian must sign treatment consent, complete psycho-social history form, child history form, child behavior checklist, given Teacher's Report Form and authorization to access patient information (when indicated).
