Road to Work Survey
This program provides medical transportation as well as pick-up and delivery of prescriptions.
The transportation services department has drivers located in Ada, Ardmore, Duncan, Purcell, Sulphur and Tishomingo.
You Provide
- CDIB
- Citizenship card
- Social Security card
- State issued ID
Eligibility
- Must reside within the Chickasaw Nation boundaries
Schedule
- 24-hour advance notice (appointments should be made between 8:30 a.m. and 2:00 p.m. Monday - Friday)
- Transportation reservations cannot be made on Chickasaw Nation recognized holidays
- 72-hour advance notice for outside of the Chickasaw Nation service area
- Transportation Services department utilizes a 24 hour 7 day a week answering service for your convenience for urgent messages

Transportation services makes Native American medical transportation comfortable, simple, safe and easy to access.
The mission of transportation services is to provide transportation and prescription pickup and delivery to all Native Americans within the Chickasaw Nation, in a comfortable, simple, safe and easy to access fashion.
