Youth Services Clothing Grant
Attention: We are pleased to inform you of changes for the youth services clothing grant program. Beginning July 1, 2012, citizens will be able to submit clothing grant applications online by using an existing user account or by creating a new user account at http://services.chickasaw.net. The user account allows user to check the status of the application. The Youth Services Clothing Grant Application (FORM #791) shown above cannot be submitted electronically, it must be printed, completed & mailed to the department of youth services.
About
The Youth Services clothing grant is funded by the Chickasaw Nation to assist eligible Chickasaw students. This grant provides students a $200 Visa gift card each school year to purchase school clothing. The grant must be used for school clothing purchases only. Grants will be provided in the form of a VISA gift card, and will be awarded once per school year. Required form is available in the forms section or address above.
Eligibility
- Eligible students must be between the ages of three to 18 years during the application period. Students who are 18 should not have completed their senior year of high school. Students age 16 or older will be required to document school enrollment or attendance.
- The application is available online or can be picked up in person or by mail at the Chickasaw Nation Division of Youth & Family Services building, 231 Seabrook Road, Ada, Oklahoma, 74820, phone (580) 272-5585.
- All applicants must be registered citizens of the Chickasaw Nation. Students possessing only a CDIB card should contact the Tribal Government Office at (580) 436-7250 to find out how to receive a citizenship card or citizenship certificate.
- Grant recipients currently possessing a VISA gift card will be notified of the award renewal by mail and/or email.
- Newly issued cards will be delivered by certified mail through the U.S. Postal Service.
- All applicants should allow four to six weeks for processing.
Deadlines
Application deadline is December 16 for the active/current school year (for example: grant applications are accepted through December 31, 2012 for the 2012-2013 school year).
Application & Award Process

Guidelines
- Grants are to be used for the purchase of clothing ONLY.
- Examples of non-clothing items include, but are not limited to, groceries, makeup, jewelry, swimwear, pajamas, hats, bags/luggage, perfume, hair products, tobacco, alcohol, fuel, etc.
- Documentation of clothing purchases is required. All original receipts must be returned to the department of youth services via mail and/or in person with the student’s name on each receipt prior to the next application period.
- Gift cards are renewable and may be usable for up to five years. Previous applicants should not discard unexpired gift cards.
- The Chickasaw Nation and the department of youth services are absolved from any further responsibility for cards upon delivery to the grantee. A lost or stolen card can be replaced for a fee of $10 that will be subtracted from the balance of the re-issued card. Lost or stolen cards should be reported immediately.
- All transactions are monitored for misuse or non-compliance. Grantees found in non-compliance will be placed on probation for the upcoming school year. Continued misuse will result in immediate suspension, for one year.
- Students who are 18 and attending a post-secondary educational institution (college or vo-tech) are not eligible for this program. These students should contact the department of education services at (580) 421-7711.
