Goal: To assist Chickasaw Citizens with special needs so that they may participate in extra curricular activities such as the Special Olympics

Vision: To promote Chickasaw Citizens with special needs to participate in a wide range of extra curricular activities that might otherwise not be possible

REIMBURSEMENT of up to $500 per year for disability related assistive technology needed or expenses incurred for a Chickasaw Citizen with special needs to participate in extra curricular activities.

• Be a Chickasaw Citizen
• Have a documented physical or mental disability

• Completed Special Needs Assistance application form
• Copy of applicant’s Chickasaw Citizenship
• Copy of applicant’s current Individualized Education Plan (IEP) or documentation of disability from a physician
• Receipts for disability related assistive technology needed or expenses paid to allow the applicant to participate in an extra curricular activity.
