The prescription mail order program provides benefits for youth, disabled citizens, elders and citizens of all ages with private insurance drug coverage to allow prescription submission from the citizen's provider. Citizens are not required to come to CNHS for medical care. Deductibles and co-payment are not collected from patients for covered medications.
To be eligible to participate in the prescription mail order program, citizens must have a CDIB and Chickasaw citizenship card. Citizens must also be 65 years of age or older; or disabled and on Medicare Part D; or youth up to the age of 18 without an active chart within CNHS. (Active chart is defined as having received services within the past three years from date of application); or citizens of all ages with private insurance drug coverage.
