The eye clinic provides examination, diagnosis and management of conditions of the eye.  Management of diabetic retinopathy and cataract surgery is available for citizens eligible for the Medical Assistance Program (MAP).  Required forms are available at the above address and all CNDH clinics.

Hours of Operation: Monday - Friday, 8 a.m. - 5 p.m.

• CDIB or proof of citzenship in federally recognized tribe
• If residing outside of service area, must provide a copy of denial letter from contract health

Eye appointments can be made either in person or by phone. When making eye appointments, all patients will be required to give their chart number or Social Security number.
