Description
The Chickasaw Nation Health System is pleased to announce the opening of the Diabetes Care Center. This facility is located in front of the Carl Albert Indian Health Facility.
This facility opened in the spring of 2004. It uses a team approach to diabetes care and is a "state of the art" facility.
Services provided at the Diabetes Care Center are:
Comprehensive Medical Management of Diabetes
Pediatric & Adult Endocrinology
Nephrology
Gestational Diabetes Management
Comprehensive Diabetes Education
Nutritional Counseling
Exercise Counseling
Retinal Photography and Ophthalmology
Foot Clinic
Dental Hygiene
Behavioral Health Counseling
Home Glucometers and Instruction

Eligibility
To be eligible for medical services at the Diabetes Care Center, one must:

Have a current chart at the Chickasaw Nation Health System, or
-
Be eligible for services and provide a copy of his/her CDIB and social security card to registration, and
-
Have a referral to the Center from a Chickasaw Nation Health System medical provider.
To be eligible for educational services at the Diabetes Care Center, one must:

Be eligible for services and provide a copy of his/her CDIB and social security card to registration,

Have the diagnosis of diabetes type I, type 2 or be insulin resistant
-
Be the family member of someone with diabetes or insulin resistance
