Diabetes Prevention Program

The diabetes prevention program is a year-long lifestyle change initiative proven to help delay and prevent type 2 diabetes. The program helps participants make lasting behavior changes involving nutrition, physical activity, and coping strategies. Classes are instructed by a trained lifestyle coach.


  • 18 years of age or older
  • BMI (body mass index) of 24 or higher
  • Must have one or more of the following:
    • Positive screening for prediabetes based on the CDC Prediabetes Screening Test
    • History of gestational diabetes mellitus (diabetes while pregnant)
    • Pre-diabetes diagnosis confirmed by a blood glucose test within the past 12 months
    • Please see enrollment questionnaire form to take screening test
