Social Security card
Proof of residency
Statement of emergency
Income or signed Non-Income Statement (available at all area offices)
Most current utility bill with a balance for verification

Chickasaw citizens with an urgent, unmet need for utility services such as gas, water, propane or electric bills and deposits can receive assistance. (October through September).
