Tribal health services support citizens with their health care needs by navigating programs, determining eligibility and finding proper health care providers and services. Assistance is available to help citizens with medical expenses when all other resources have been exhausted. The program can help with unmet medical services, dental services and durable medical equipment.

Applicants must be a Chickasaw citizen and treatment must be medically necessary. Additional eligibility requirements may apply.

Eligibility
  • Chickasaw citizen with a Chickasaw Nation citizenship card
  • Service must not be available through Chickasaw Nation Department of Health or IHS facilities
  • Must be medically necessary
  • Must not be from a self-inflicted injury, including drug or alcohol related
  • Must have exhausted other resources such as insurance, Medicare and Medicaid or any other responsible party
You Provide
  • Copy of insurance (if applicable)
  • Treatment plan with estimate of cost, script and/or referral
  • If referred by Chickasaw Nation Department of Health or Indian Health Service (IHS) facility, provide a copy of denial letter from IHS purchased referred care
  • Original invoice and explanation of benefits (EOB)

Chickasaw citizens can use this resource to navigate programs, determine eligibility and receive assistance with finding health care providers and services. Tribal health staff is available to answer questions and help citizens access health care.

Resource Assistance
This service is designed to assist citizens with medical expenses when all other resources have been exhausted and to serve as a supplement to contract health services that provide necessary medical services for the ongoing care from the Chickasaw Nation Department of Health (CNDH) to maximize medical care.

The program assists tribal citizens with unmet medical services, dental services and durable medical equipment (DME) up to $5,000 annually.

Tribal health staff is available to assist citizens with questions and help citizens navigate their health care.

The Chickasaw Nation Department of Health offers a variety of programs and services at several convenient locations throughout our 13-county jurisdictional boundaries. The Chickasaw Nation Medical Center (CNMC) is located in Ada, Oklahoma, and outlying clinics are located in Ardmore, Purcell and Tishomingo. The program does not cover services that are provided within the CNDH.
