Optometry Services


Out of an abundance of caution and to mitigate the spread of the coronavirus, or COVID-19, certain Chickasaw Nation events, classes, facilities, etc., will be postponed, canceled or closed. An integral part of the Chickasaw Nation Department of Health, optometry provides services in the prevention, diagnosis and treatment of eye disease and the promotion of eye health. When provided early, other eye care services and activities can include ocular disease processes, to prevent further visual impairment or maintain quality of life. An on-site retinal clinic includes ophthalmologist services and a general ophthalmologist.

  • Birth Certificate (only if born out of state)


  • Eye examination for children ages birth-18
  • Eye examination for adults 19-55+
  • Chickasaw or Native American
